When you arrive, you will be asked to fill out several forms, including a patient information form and a medical history form. For your convenience, we have included the forms in a PDF format for you to download, complete, and bring with you to your first appointment.

For some patients, the first visit is an emergency. We reserve a limited amount of time each day for emergency patients and within the time allotted we will diagnose the situation, make you as comfortable as possible, and schedule further treatment as necessary.

Good dentistry is more than excellent clinical skills and state of the art equipment. Establishing lasting relationships with our patients based upon mutual trust and open communication is important to us. One of our goals is to establish a long-term result that looks great, feels comfortable, and fits your dental budget. We will work with your insurance company to ensure that you are able to take full advantage of your benefits.
Please note that typically a "cleaning" is not done during the first visit. Experience has shown us that each patient's needs are different. We cannot anticipate what type of cleaning a patient will need or how much time is required without an examination. In order to provide the best treatment possible, the appropriate amount of time will be reserved for preventive care.
Our overall goal is to provide to you, our patients, the highest quality of dentistry obtainable in an atmosphere that is both relaxed and professional. We welcome your questions at your appointment and look forward to seeing you.
